Thank you for joining our workshop! Before participating, please read and sign this agreement. This waiver helps ensure everyone understands the risks involved and how to safely enjoy the experience.

1. Acknowledgment of Workshop Risks

I understand that participation in this jewelry-making workshop involves the use of materials, tools, and equipment, including but not limited to metal clay, small tools, gemstones, and kiln/firing equipment (primarily handled by the instructor).

I acknowledge that participation involves certain inherent risks, including but not limited to minor cuts, burns, allergic reactions, injury from tools or materials, or other unforeseen risks. I voluntarily assume all risks associated with participating in this activity and agree to follow all safety instructions provided by the instructors.

I confirm that I am physically able to participate and will inform the instructors of any medical conditions, allergies, or concerns that may affect my safety during the workshop.

2. Pet Accessories — Decorative Use Only

I acknowledge that any beaded necklace, collar charm, or similar accessory created during this workshop is intended for decorative purposes only and is not designed, tested, or intended to function as a collar, harness, leash attachment, or replacement for an identification tag or safety device.

I understand that pet accessories containing small components (including but not limited to beads, clasps, jump rings, and findings) may present potential risks, including choking, strangulation, ingestion, or injury if chewed, pulled apart, damaged, or worn unsupervised.

I agree that:

• I will only allow my pet to wear the accessory while directly and continuously supervised by a responsible adult.
  
• I will remove the accessory before unsupervised time, play with other animals, sleeping, crating, bathing, travel, or any activity where the accessory could become caught or damaged.
  
• I will regularly inspect the accessory for wear, loose components, or damage and discontinue use immediately if any issues are found.
  
• I am solely responsible for determining whether the accessory is appropriate for my pet’s size, temperament, and behavior.
  

3. Release of Liability

In consideration of being allowed to participate in this workshop and/or create a pet accessory, I voluntarily release and hold harmless Studio Bubby, Bright Greys Studio, their owners, instructors, assistants, partners, and the workshop venue (Little Lions) from any and all claims, damages, liabilities, losses, or expenses (including veterinary expenses) arising from or related to my participation in the workshop or my pet’s use of any accessory created during the workshop, except to the extent caused by gross negligence or willful misconduct.

4. Photo & Media Release

I grant permission for Studio Bubby and its partners to take photographs and/or videos during the workshop and use these images for promotional purposes, including social media, websites, and marketing materials.

If I do not wish to be photographed or recorded, I will notify the instructors before or during the workshop.
